Case Manager

Job Title: Case Manager
Reports to: Program Director 
Schedule: Full Time, 40 hours per week, Monday - Friday

The Case Manager is responsible for providing comprehensive case management services to individuals experiencing homelessness. This role focuses on supporting residents in achieving stability, housing, and self-sufficiency through individualized service planning, coordination of community resources, and ongoing advocacy. The Case Manager works closely with a multidisciplinary team and plays an active role in resident engagement and community collaboration.

Essential Duties and Responsibilities
• Manage a fluctuating caseload of approximately 25–30 residents, adjusting support levels based on individual needs and program expectations.
• Complete assessments, individualized service plans, and regular progress reviews with residents.
• Facilitate resident meetings, goal-setting sessions, and educational or support-based discussions.
• Maintain accurate, timely, and confidential documentation in accordance with agency policies and funding requirements.
• Collaborate effectively with shelter staff, case managers, and leadership to ensure coordinated and consistent service delivery.
• Attend and actively participate in community meetings, case conferencing, and partner collaborations with external agencies.
• Advocate for residents by connecting them to housing resources, employment services, healthcare, benefits, and other community supports.
• Monitor resident progress, address barriers, and provide crisis intervention and problem-solving support as needed.
• Uphold shelter policies, promote resident accountability, and model respectful, professional boundaries at all times.

Qualifications
• Previous experience working with individuals experiencing homelessness or other vulnerable populations strongly preferred.
• Ability to manage a dynamic and sometimes high-pressure caseload while remaining organized and resident-focused.
• Strong communication skills, both written and verbal.
• Proven ability to work collaboratively as part of a team.
• Comfort facilitating group meetings and engaging residents in a supportive yet structured manner.
• Knowledge of community resources, housing systems, and social service networks is a plus.
