Case Manager

Job Title: Case Manager
Reports to: Program Director 
FLSA: Non-Exempt

Summary: Responsible for the case management of a portion of the residents of the New Community Shelter Programs and for a portion of the programming facilitation/coordination. 

  •  Essential Duties and Responsibilities include the following:
    -Ability to efficiently and effectively case manage approximately 20-25 individuals per 40-hour position (or more as determined to be necessary by the CEO).
    -Understanding and utilization of professional boundaries, ethics, and confidentiality.
    -Knowledge of and experience in working with people with a wide spectrum of individual issues, including but not limited to: AODA concerns, mental health concerns, and physical/mental disabilities.
    -Ability to access community resources and maintain relationships with community based service providers (i.e. Salvation Army, Brown County Human Services, Wisconsin Job Center, etc.)
    -Collaborate with Housing providers to ensure a complete continuum of care services.
    -Knowledge of assessment and evaluation process including problem and strength identification.
    - Ability to determine residents’ needs, establish individual goals and action plans, hold the residents accountable to their goals and action plans, monitor and evaluate progress, make appropriate referrals, initiate interventions,   determine appropriate deadlines, and establish follow-up support/plans with each individual served.
    -Ability to make decisions and work unsupervised in supporting the needs of the residents.
    -Ability to provide crisis counseling/management and make independent decisions.
    -Ability to effectively collaborate with House Supervisors and volunteer staff to maximize services provided.
    -Effectively network and advocate on behalf of all residents with all service providers, government units, and community based organizations.
    -Ability to maintain accurate and complete resident case files and stay current with data entry and statistical requirements.
    -Ability to utilize a computer and basic programs such as Microsoft Word, Excel, Access, PowerPoint, and Outlook for communication, documentation, and data entry.
    -Ability to organize time.
    -Ability to speak publicly.
    -Ability to facilitate and co-facilitate various educational and support groups offered within the programming of the New Community Shelter. Including but not limited to Employment Readiness, Rent Smart, and Mandatory Resident/Tenant Meetings.
    -Attend staff and committee meetings as requested and/or necessary.
    -Make recommendations to Program Director and CEO regarding discontinuation of services, provision of services not available, or other resident related needs.
    -Other duties may be assigned by the Program Director, and/or CEO.

Supervisory Responsibilities: Peer supervision of other Case Managers.

Education and/or Experience: Human service related 4 year B.S. degree, or the equivalent combination of education and work experience. 

Language Skills: Ability to read, analyze, and interpret professional communication, technical procedures, and government regulations.  Ability to write case notes, professional correspondence, and reports.  Ability to effectively present information and respond to questions from groups of team members, residents, and the general public.   

Mathematical Skills: Ability to calculate figures and amounts. Ability to apply concepts of basic algebra and geometry.  Ability to assist residents in establishing, evaluating, and maintaining a personal budget.

Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions.  Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables.

Certifications, Licenses, Registrations: Must hold a valid driver’s license, registration, auto insurance and have an auto accessible, as needed. If using the title “social worker” must possess a current and valid certification from the State of Wisconsin Department of Regulation and Licensing.
